Welcome to the STC Communication Wellness Survey

Everyday it becomes more evident that there is a need for early detection and intervention in the areas of speech and language. Although, there are many resources available to children with special needs, these resources are sometimes not readily available or parents are sometimes unaware that they exist and as a result there are still many children that fall through the cracks.

The purpose of this tool is to provide a basis for early detection of children that are in need of intervention in the area of communication as well as to educate parents on why's and how's of speech and language services.

Please take some time to answer the following questions. Each question is specifically tailored to detecting areas related to speech, language, and communication that your child may be experiencing difficulty with.

Once you have finished you will receive results denoting if there is a need for intervention and/or misconceptions about speech and language development that may lead a parent to think there is a need for intervention when there is most likely none.

Keep in mind that only "yes" answers will denote a point to your score therefore a "no" answer is equal to a "not sure" or "not applicable." Please note that this survey is a comprehensive one made up of 40 questions and not all questions may apply to your particular case. Please do worry of the question does apply to your child, it will not affect the score, simply answer "no."

The questions are very scattered and unrelated, but they are meant to be this way in order to detect issues in all areas that a speech pathologist can treat.

Are the child's teeth completely aligned when they smile or do they have a open bite? (Orofacial Myofunctional)

When your child sticks out their tongue, does it form a heart shape instead of a point? (Orofacial Myofunctinal)



Has your child suffered a fall and/or accident to the head, high fevers and/or seizures? (Medical Diagnosis)



Did or does your child suffer from frequent ear infections or has he or she been diagnosed with a hearing impairment? (Medical Diagnosis)



Does your child have difficulty chewing or eating? (Feeding and Swallowing)



Is your child a picky eater? (Feeding and Swallowing)



Does your child use toys appropriately while playing, i.e., holds a toy telephone to his/her ear as opposed to banging it or attempting to bounce it like a ball? (Play Skills)



Does your child show disinterest when other kids are around? (Social Skills)



Does your child prefer to play alone? (Social Skills)



Does your child have difficulty following commands and directions? (i.e. "Bring me your shoes" or "Wave goodbye") (Receptive Language)



Do you feel that your child never listens to you? (Receptive Language)



Does your child respond to simple questions appropriately (i.e. "What sound does the cow make?") (Receptive Language)



Does your child communicate with you by either gestures or pointing, hand over hand directing, or true words mixed with jargon, as if he/she had their own language? (Expressive Language)

Do people who are unfamiliar with your child have difficulty understanding him/her when speaking? (Speech Intelligibility)



Do you feel that your child is communicating his/her wants and needs well; however is not pronouncing the words correctly? (Articulation and Phonology)



Is your child belos the age of 5 and produces a lisp when he/she speaks or has difficulty with the pronunciation of the the follwoing sounds: "L" "R" or "S" (for example, "thoap" for "soap")? (Common Concerns)



Is your child's tongue in a forward position in the mouth when eating, speaking, or swallowing? (Orofacial Myofunctional)



When you ask your child to repeat what you say, is he/she unable to do so? (Apraxia)
